Advice needed on filling of Form 14 for Immigration Act

Hi all, my GF (PRC) was sent back last Oct and I want to apply for her to come over again.
Went ICA to see the Officer n was given the above to fill.
Anyone can guide me thru it thru pm?

Visitor Visa n Visitor Entry

Relationship put wat better? Boyfriend or Friend?

And also, how to write the letter of appeal?
